A giant bladder stone in combination with a stone necklace of urinary tract in patient with nephrotic proteinuria and concurrent bladder and prostate neoplasia

We present a case report of a 66-year-old homeless man who came to our observation for acute kidney failure, urinary tract infection, nephrotic proteinuria and dehydration. The instrumental examinations revealed multiple bladder stones with formations of dimensions rarely observed in literature. Cystotomy allowed to remove not only multiple stones (weighing a total of 550 grams) but also a ureteral stent, positioned many years earlier, which had formed the core around which the lithiasis had formed. Biopsy of the bladder’s left lateral wall revealed squamous cell carcinoma. He underwent radical cystectomy and prostatectomy, bringing to light prostate adenocarcinoma.
