Therapeutical apheresis (TA) has been successfully used in the treatment of exacerbation of demyelinating central nervous system (CNS) diseases that have not responded to first-line treatment with corticosteroids. Nevertheless, these evidences need to be further confirmed according to the American Academy of Neurology guidelines. The main body of data concerns multiple sclerosis and neuromyelitis optica, which respectively involve mainly the T and B-cell CNS immune responses. TA is ineffective in chronic or secondary progressive multiple sclerosis.
